- Position Summary:
Within Dignity Health Medical Foundation, Primary Care Centers utilize the RN team-based model of care. The Primary Care RN is an integral part of the care team and performs patient care services that support the team-based care model. The Primary Care RN is expected to know his/her scope as defined by the California Business and Professional Code and function at the top of his/her licensure. - The Primary Care RN works in collaboration with physicians, staff, clinic management and other healthcare professionals in a primary care medical home, in a team approach with continuous partnership with physicians and patients to promote timely access to needed care and the enhancement of patient and family well-being.
- The Primary Care RN uses protocol-driven standing orders to perform comprehensive support to physicians consistent with scope of practice, which may include evaluating lab data, providing instruction and education to patients, phone triage, and medication modifications.
- Provides coordination of care to support disease management education, facilitate diagnostic and laboratory testing, ensure timely scheduling of follow-up appointments with providers, and contribute to the ongoing development and process improvement of the clinical point of care program within primary care.
- Provides leadership and guidance to LVNs, MAs to ensure practice at the top of licensure within scope of practice.
- Coordinates resources to facilitate the patient’s progress through the continuum of care and provides patient education, disease management and population health services in support of physicians managing patients with chronic diseases.
- Coordinates a wide range of patient self-management support and disease registry activities and works with the extended care team to ensure smooth transitions across the continuum of care.
- Collaborates with the Ambulatory Care management program to refer patients who require intensive care management and ensures smooth transitions between clinic care teams and complex care teams.
- Minimum Qualifications:
- Three (3) years’ experience as an RN. A minimum of one (1) year in the last 5 years as an RN in an ambulatory community involving care management and/or chronic disease management (including outpatient community health clinics, private physician offices, home health and hospices, public health settings, free-standing urgent care clinics) or primary care setting
- Graduate from an accredited school of nursing required.
- Clear and current CA RN license.
- Understanding of patient centered care, team-based care and evidence-based clinical practice guidelines.
