Registered Nurse Case Manager:
Home Health
Pay Range: $130,000 – $160,000 per year. Additional incentive bonuses up to $10,000 annually.
Full‑Time Registered Nurse Case Manager. Part‑time opportunities available.
Territory:
Citrus Heights, Fair Oaks, and surrounding areas.
- Utilize clinical skills to create, implement, and execute comprehensive care plans to promote wellness and prevent rehospitalization.
- Conduct full assessment of patient and family upon initial visit.
- Develop advanced care plans including rehabilitation using assessment data.
- Provide education and support in wound care, blood draws, tube feeding, tracheostomy, Foley catheters, CHF management, and medication education.
- Case‑manage patient care episodes with an interdisciplinary clinical team.
- Prepare clinical notes, orders, and charting within compliant time frame.
- Collaborate with physicians and agency staff to deliver optimal care.
- Initiate and administer IV medication infusions safely and effectively in accordance with protocols.
- Current and unrestricted California RN license.
- Current BLS/CPR certification.
- Valid California driver’s license.
- High level of clinical knowledge in home health care (high acuity preferred).
- Experience with interdisciplinary teams and case management fundamentals.
- Prior IV medication infusion experience preferred.
- Strong clinical knowledge and critical thinking; self‑directed, empathetic, and organized.
- Excellent interpersonal and leadership skills.
