Registered Nurse Case Manager:
Home Health

Pay Range: $130,000 – $160,000 per year. Additional incentive bonuses up to $10,000 annually.

Full‑Time Registered Nurse Case Manager. Part‑time opportunities available.

Territory:
Citrus Heights, Fair Oaks, and surrounding areas.

Responsibilities
  • Utilize clinical skills to create, implement, and execute comprehensive care plans to promote wellness and prevent rehospitalization.
  • Conduct full assessment of patient and family upon initial visit.
  • Develop advanced care plans including rehabilitation using assessment data.
  • Provide education and support in wound care, blood draws, tube feeding, tracheostomy, Foley catheters, CHF management, and medication education.
  • Case‑manage patient care episodes with an interdisciplinary clinical team.
  • Prepare clinical notes, orders, and charting within compliant time frame.
  • Collaborate with physicians and agency staff to deliver optimal care.
  • Initiate and administer IV medication infusions safely and effectively in accordance with protocols.
Qualifications
  • Current and unrestricted California RN license.
  • Current BLS/CPR certification.
  • Valid California driver’s license.
  • High level of clinical knowledge in home health care (high acuity preferred).
  • Experience with interdisciplinary teams and case management fundamentals.
  • Prior IV medication infusion experience preferred.
  • Strong clinical knowledge and critical thinking; self‑directed, empathetic, and organized.
  • Excellent interpersonal and leadership skills.
