Job DescriptionThis job optimizes patient care by promoting rational, safe and cost effective pharmaceutical care including drug dispensing, intravenous admixture program and providing drug information. Conducts drug therapy monitoring and implements drug therapy interventions to reduce adverse events. Oversees the work performed by pharmacy technicians, students and other support staff.
Essential Responsibilities- Process prescriptions:
Conduct prospective drug review on all prescriptions processed. Independently review and assess medication prescriptions to identify, prevent and resolve potential and actual drug related problems; ensure accurate and efficient compounding and dispensing. Ensure safe, appropriate and cost-effective medication therapies for patients. Demonstrate sound professional judgment and knowledge to resolve emergent issues. (20%) - Drug information and patient interventions:
Provide accurate and timely drug information to other health professionals, patients and the public. Document interventions and outcomes related to drug therapy. Collaborate with the healthcare team to manage drug therapy, protocols and guidelines. (20%) - Assist in department management:
Assist the manager in supervising and directing work of pharmacy technicians, students, externs and other support personnel in daily operations. Train pharmacists, pharmacy technicians, pharmacy residents and students in their duty areas. Oversee daily inventory maintenance purchases. Ensure controlled substances are stored and dispensed in compliance with regulations. (20%) - Work environment:
Contribute to effecting the department through computer functions, refill medications, generation of labels and answering phones appropriately. Maintain a clean and orderly work environment. (20%) - Safety:
Participate in performance improvement and regulatory compliance efforts. Maintain an attitude that promotes and fosters an environment of safety. All steps performed in a courteous and efficient manner. (20%) - Night pharmacist responsibilities:
May be responsible for triaging, prioritizing and delegating nightly activities of the pharmacy department. Serve as primary interface to resolve medication related problems between pharmacy, nursing and other disciplines. Demonstrate proficiency in troubleshooting and solving IT, computer and automation problems. - Perform other duties as assigned or required.
- Minimum Bachelor’s degree in Pharmacy or Doctor of Pharmacy degree (Pharm
D) - Minimum Licensed or eligible for licensure as a Registered Pharmacist in the state of PA at hire or sit for boards and obtain licensure within 120 days.
- Preferred Hospital experience preferred.
