We are seeking a dedicated Neighborhood/Household Charge Nurse (LVN) (PM Shift) to join our team in a on-call  this role, you’ll provide hands-on nursing care, support a resident-directed culture, and serve as a teacher and role model for neighborhood/household staff, ensuring high-quality, individualized care for every resident.##

** What You’ll Do
*** Administer medications and treatments safely and according to plan of care.
* Observe residents’ physical and emotional condition and report changes to the Nurse Supervisor (RN), Director of Nursing, or attending physician.
* Collaborate closely with the Nurse Leader to ensure regulatory compliance and best practices.
* Support a person-centered, resident-directed environment that fosters independence and choice.
* Serve as a mentor and role model for neighborhood/household staff, promoting professional nursing practices.
* Provide routine daily nursing care and services aligned with each resident’s plan of care and personal preferences.
* Advocate for residents’ needs and contribute to a safe, supportive, and homelike environment.## ##

** What You Bring
*** Graduate of an accredited school of professional nursing.
* Current, active California Licensed Vocational Nurse (LVN) license.
* Minimum of one year of experience in a skilled nursing or senior living setting preferred.
* Ability to read, write, and communicate effectively in English.
* Strong clinical judgment, observation, and documentation skills.
* Commitment to fostering a resident-centered, collaborative, and learning-focused team environment.
