LVN​/LPN – LTC​/SNF

Job Summary:

This role involves providing high-quality nursing care to residents in a long-term care or skilled nursing facility. The LVN/LPN will support the healthcare team in delivering compassionate, patient-centered services during each shift.

Responsibilities:

• Administer medication and perform basic nursing procedures in accordance with state and facility guidelines.
• Monitor residents’ conditions and report any changes to the supervising RN or healthcare team.
• Document patient care accurately and maintain detailed, compliant records.
• Assist with patient assessments, including vital signs and symptom observations.
• Support residents with daily living activities and ensure their safety and well-being.
• Adhere to facility policies regarding infection control, including COVID-19 protocols and identification requirements.

Qualifications:

• Valid LVN or LPN license issued by the state.
• Proven experience in long-term care or skilled nursing facilities preferred.
• Ability to perform clinical procedures with accuracy and compassion.
• Strong communication skills to collaborate effectively with team members and residents.
• Flexibility to work three 12-hour shifts per week, including every other weekend.
• Commitment to following safety protocols and documentation standards.
