Minimum Qualifications
  • Current Registered Nurse license issued by the state in which services will be provided or current multi‑state RN license through the enhanced Nurse Licensure Compact (eNLC).
  • Valid driver’s license.
  • CPR certification within 30 days of hire date.
Preferred Qualifications
  • One year of Home Care Nursing and 2 years of prior nursing experience.
Core Duties and Responsibilities
  • Perform hospice aide functions and monitor basic patient care according to policies.
  • Provide a safe and comfortable environment in the patient’s home.
  • Ensure privacy and demonstrate concern for the patient’s dignity during all care.
  • Complete assignments promptly and assume on‑call responsibility per policy.
  • Communicate patient care problems to Administrative Staff and IDT members to assure continuity and coordination.
  • Provide care based on physical, psychosocial, communication, safety, education level, and related criteria for all age groups.
  • Utilize admission criteria per agency guidelines and formulate a plan of care with the IDT.
  • Document full system assessments, including physical and psychosocial aspects, and identify age‑specific criteria upon admission and each visit thereafter.
  • Respond to changes in patient condition with appropriate nursing action and report significant changes to staff or physician.
  • Document patient status and services furnished in accordance with documentation policy.
  • Perform CPR when directed by patient’s representative’s wishes.
  • Use and maintain DME and medical supplies efficiently; notify admin/doctor if R/T DME issues occur.
  • Demonstrate proper bag technique and equipment maintenance per policy.
  • Utilize supplies effectively to ensure cost containment and maintain IV site integrity.
  • Perform treatments/procedures (e.g., vital signs, specimen collection, ostomy care, catheterization, wound care, suctioning, gastric tube maintenance).
  • Provide patient teaching according to policy.
  • Coordinate medication administration and changes in level of care according to standards.
  • Identify stages of death and dying.
  • Complete IDT forms, Medicare LMRPs, and patient discharge summaries per regulations.
  • Perform evening and weekend duties, including chart audits and follow‑up calls as needed.
Physical Requirements
  • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling.
  • Lift, move, and position patients of all weights with varying assistance.
  • Maintain visual acuity and hearing within normal ranges.
  • Exert 10–50 pounds of force occasionally or frequently as needed.
  • Operate a motor vehicle safely.
Working Environment
  • Exposure to bodily fluids, human tissues, contagious diseases, sharps, and toxic gases.
  • Exposure to electrical current and radiation from X‑rays.
  • Exposure to unpredictable road and weather conditions during patient visits.
  • Work primarily in patients’ homes with extensive travel required.
Skills and Abilities
  • Perform complex mental activity in highly technical situations.
  • Work successfully under high stress, adapting to varying workloads.
  • Make sound independent judgments and collaborate with multidisciplinary team members.
  • Comprehend and follow oral and written instructions.
  • Possess strong written and verbal communication skills.
  • Calculate drug dosages and infusion rates accurately.
  • Work weekends, holidays, and all shifts.
  • Demonstrate knowledge of and compliance with safety policies.
  • Perform IV skills and other nursing procedures.
