Milestone LLC is currently seeking a compassionate and detail-oriented Community Support Specialist to join our team. In this vital role, you will act as a liaison between clients and various services, ensuring that individuals receive the support they need to navigate their personal challenges and achieve their goals. As a CSS, you will be responsible for developing individualized service plans and coordinating resources to address the unique needs of each client.

You will work closely with clients, their families, and community providers to offer guidance, support, and advocacy while promoting self-sufficiency and empowerment. Responsibilities
  • Conduct assessments to identify client needs and develop comprehensive service plans.
  • Facilitate access to community resources and services, including healthcare, housing, and employment.
  • Provide ongoing support and guidance to clients, regularly monitoring their progress.
  • Coordinate care among various providers and agencies to ensure a holistic approach to client support.
  • Maintain accurate records and documentation for each client in compliance with agency policies and regulations.
  • Advocate for clients to enhance their access to services and support systems.
  • Engage in outreach and community education to raise awareness about available resources.
  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
  • Previous experience in case management, social services, or a related area preferred.
  • Strong understanding of community resources and support services.
  • Excellent communication and interpersonal skills for effective client interactions.
  • Ability to work collaboratively with clients and multidisciplinary teams.
  • Strong organizational skills and attention to detail in case documentation.
  • Commitment to client confidentiality and ethical standards in practice.
