Center Well Home Health is seeking a licensed Physical Therapist to deliver in-home therapies, helping patients regain strength, mobility and independence through personalized treatment plans.
Reporting to the Clinical Manager, you will assess patient needs, document progress, supervise PTAs and Home Health Aides, and coordinate with physicians and family to achieve functional goals. Current PT license and CPR certification are required; one year of experience is preferred.
