As a Tailored Care Manager
, you’ll evaluate client needs, develop personalized care plans, and coordinate services addressing physical and mental health, social determinants, and life challenges. This role offers travel throughout Johnston and surrounding counties to close gaps in care.

Earn a competitive salary of $52,000‑$54,000 annually, based on experience. This full‑time position typically runs Monday–Friday, 9 AM–5 PM, with rotating on‑call responsibilities. We value work‑life balance and provide a supportive environment.

We seek a compassionate, driven professional willing to provide services in various community settings and participate in rotating after‑hours support.

• Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services area (Master’s preferred)
• Eligibility for Qualified Mental Health Professional (QMHP) credentialing
• Minimum 2 years of experience with mental health and/or substance use populations
• Proficiency in Microsoft Office and Electronic Health Records (EHR)
• Valid driver’s license, current auto insurance, and clean driving record
• Strong communication skills and ability to work independently
