RN / LPN - Staff Nurse & Care Coordinator (Dual Role) Float Clayton/Cary

We’re seeking an energetic, organized, and mission‑driven RN or LPN to take on a dual role as Staff Nurse & Care Coordinator in our Clayton and Cary locations. Responsibilities- Assisting providers with locating, analyzing, and summarizing clinical patient information.
- Locating community resources that benefit staff, patients, and families.
- Helping patients and their care team find ways to overcome barriers.
- Developing close relationships with patients and families, especially complex cases, to secure necessary resources and care.
- Assisting providers and staff with identifying and closing gaps in patient care, and following up to ensure needs are met.
- Working with providers on developing and maintaining patient care plans for medically complex patients.
- Reviewing, documenting, and following up with patients seen by the ED, admitted to the hospital, or visited Urgent Care.
- Cultivating and maintaining relationships with specialists and referral agencies; becoming the primary contact for hospitals, specialists, and healthcare agencies.
- Coordinating transition to adult care for young adult patients, especially those with complex medical or psychosocial histories.
- Assisting clinical staff with clinic nursing duties, telephone triage, and prescription refill calls.
- Valid RN or LPN license in the state of North Carolina.
- Proficient in patient assessment and care.
- Knowledge of EHR systems and HIPAA regulations.
- Basic life support certification required.
- Strong communication skills and ability to work with diverse patient populations.
