RN / LPN - Staff Nurse & Care Coordinator (Dual Role) Float Clayton/Cary

We are seeking an energetic, organized, and mission‑driven RN or LPN to take on a dual role as Staff Nurse & Care Coordinator in our Clayton and Cary locations.

- Be the nurse detective. Assist providers with locating, analyzing, and summarizing clinical patient information; locate community resources that benefit staff, patients, and families; help patients and their care team find ways to overcome barriers.
- Be the advocate. Develop close relationships with patients and families, especially with complex patients; assist with securing the resources and care needed to keep patients healthy; identify and close gaps in patient care; follow up with patients/families to ensure their needs are met; work with providers on developing and maintaining care plans for medically complex patients; review, document, and follow up with patients seen by the ED, admitted to the hospital, or who have been to urgent care.
- Be the connection. Cultivate and maintain relationships with specialists and other referring agencies; become the primary contact for hospitals, specialists, and other healthcare agencies; coordinate transition to adult care for young adult patients, especially those with complex medical or psychosocial histories.
- Be a team player. Assist clinical staff with clinic nursing duties, telephone triage, and prescription refill calls.
- Valid RN or LPN license in the state of North Carolina
- Proficient in patient assessment and care
- Knowledge of EHR systems and HIPAA regulations
- Basic Life Support certification required
- Strong communication skills and ability to work with diverse patient populations
