Job Summary:As a Health Information Manager, you will play a key role in keeping participant health records accurate, secure, and accessible. You’ll support clinical and administrative teams by ensuring documentation is complete, compliant, and aligned with PACE program standards. Your work helps every department deliver safe, coordinated, high-quality care.

Schedule:First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Health Information Management & Compliance
- Develop, implement, and maintain policies and procedures for health information management in alignment with PACE regulations and HIPAA standards.
- Ensure confidentiality, security, and integrity of all participant health records (electronic and paper).
- Monitor compliance with CMS, state, and organizational requirements for documentation and reporting.
- Conduct regular audits of medical records for accuracy, completeness, and timeliness.
- Manage and optimize EHR workflows to support interdisciplinary care planning and regulatory reporting.
- Train staff on proper documentation practices and system use.
- Troubleshoot EHR issues and coordinate with IT for system enhancements.
- Oversee coding and classification of diagnoses and procedures for accurate billing and reporting.
- Generate and analyze reports for quality improvement, risk management, and regulatory submissions.
- Support data collection for PACE quality measures, UDS reporting, and other performance metrics.
- Work closely with providers, nurses, care coordinators, social workers and all PACE staff to ensure timely documentation of assessments, care plans, and transitions of care etc.
- Serve as a resource for clinical teams regarding documentation standards and regulatory requirements.
- Implement and monitor disaster recovery plans for health information systems.
- Investigate and resolve data integrity issues and privacy breaches in accordance with policy.
