We are seeking a registered nurse with home health and hospice experience to join our team at Harmony Home Health & Hospice in Clearfield, UT. This role offers the opportunity to provide compassionate care to patients in the comfort of their own homes, while being part of a supportive and dedicated team. This is NOT a 7 on 7 off on call position.

We are seeking someone available to take on call 1-2 days per week and 1-2 weekends per month for a small census.

• Provide skilled nursing care to patients in their homes
• Assess patient needs and develop individualized care plans
• Administer medications and treatments as prescribed by the physician
• Collaborate with the interdisciplinary team to ensure comprehensive care for patients
• Provide education and support to patients and their families

• Current RN license in the state of Utah
• Minimum of 2 years of nursing experience, preferably in home health or hospice
• Strong clinical assessment and critical thinking skills
• Compassionate and empathetic demeanor
• Ability to work independently and as part of a team
