LHCR is seeking an experienced MDS Coordinator, RN/LPN, to join our skilled nursing and rehabilitation team in Clearlake, CA. You will lead MDS data collection, ensure regulatory compliance, and work with the Nurse Leadership Team to place residents first in daily care decisions.
You must hold an unencumbered CA RN license, have 1–3 years of leadership experience, and be a graduate of an accredited College of Nursing with current BLS. On-call rotation is required.
