Reporting to the Clinical Care Manager, the CNA is responsible for assisting with direct patient care within the scope of their practice. The CNA provides personal care to patients and assists with activities of daily living in the patient’s home, facilities, and/or inpatient units. The CNA maintains their own clinical and mandatory education requirements, meets the established standards of their profession, and is an advocate for patients, families, and their profession.
The CNA demonstrates knowledge of age‑specific patient populations and cultural diversity during the provision of patient care, contributes to the provision of quality care through performance improvement techniques, promotes an environment of care, and supports the organizational vision and mission.
- Current certification in the State of Florida as a Certified Nursing Assistant.
- A minimum of one (1) year of experience preferred.
- Must maintain current CPR certification.
- Experience caring for terminally ill patients.
- Experience caring for patients with life‑threatening illness.
- Valid Florida Driver’s License.
- Valid auto insurance.
Job Responsibilities
- Performs or assists patients with tub/shower/bath, complete/partial bed bath, personal care, skin care dressing, nail care, hair care, foot care and oral hygiene.
- Take and record blood pressure, pulse, respiration, weight and temperature and report abnormalities to the RN.
- Assist patient or caregiver with the use of Hoyer lift, commode, bedpan, urinal, wheelchair, hospital bed and other equipment as directed by a registered nurse.
- Assist patients to achieve maximum independence in activities of daily living and promote optimal well‑being for the patient by maintaining a clean, safe, and healthy environment.
- Make beds; change linens.
- Assist with positioning, transfers, ambulating, and activity as directed by the RN.
- Follow established Infection Control and Universal Precautions policies and procedures when performing daily tasks.
- Utilize proper body mechanics while lifting and moving patients, and during other activities.
- Record patient’s health status and services provided each visit in an informative and descriptive manner.
- Immediately report all changes in the patient’s condition, accidents and incidents to the Case Manager or NCM.
- Report all complaints and grievances made by the patient to the Case Manager or Clinical.
- Actively participate in interdisciplinary group (IDG) by informing RN Case Manager of changes/status.
- Maintain a professional relationship and serve as a liaison with other providers.
- Offer emotional support to the patient/family in accordance with the hospice philosophy and plan of care.
- Represent Gulfside in patient interactions and maintain a positive attitude.
- Maintain a positive attitude and display a “can‑do” attitude.
- Show up for work as scheduled and consistently demonstrate dependability and punctuality.
