Reporting to the Clinical Care Manager, the RN Hospice Field is responsible for providing quality care to patients in private residences, assisted living facilities, and skilled nursing homes. The RN Hospice Field utilizes assessment, intervention, and education to maximize patient comfort and enhance quality of life for the patient/family. The RN Hospice Field oversees and case‑manages the needs of patients assigned to them and provides supervision for LPNs, CNAs, and other staff in the field.
They maintain their own clinical and mandatory education requirements and act as an advocate for patients, families, and the profession.
$7,500 Sign‑On Bonus
Shift: Saturday & Sunday 8am - 12am
Education and Qualifications- Currently licensed in the State of Florida as a registered nurse.
- Minimum of one (1) year of nursing experience in a hospital, home health, or hospice setting (preferred).
- Strong ethical standards and professional demeanor.
- Sound critical thinking, time management, technical skills, analytical ability, and good judgment.
- Well‑organized, self‑directed team player.
- Reliable transportation, valid Florida driver’s license, and proof of automobile insurance.
Job Responsibilities
- Case‑manage the needs of patients and families assigned in the field.
- Perform ongoing assessment of physical, psychological, social, and spiritual needs of the patient/family unit and initiate appropriate intervention.
- Provide and/or oversee supervision of P.N.’s, C.N.A.’s, and other staff.
- Takes independent action to identify problems.
- Support policies and procedures.
- Assist in the orientation of new staff members.
- Serve as a resource person; provide mentoring and guidance for co‑workers and additional staff.
- Maintain positive working relationships with team members of all disciplines.
- Attend to patient/family complaints and concerns.
- Initiate and promote initiatives to develop and strengthen relationships with facility partners.
- Take a proactive approach to identifying and resolving potential issues.
- Educate the community regarding hospice care, philosophy, and concept through individual consultation with community members.
- Communicate regularly with the Clinical Care Manager and continuously assess areas for risk management.
- Keep current with organizational and departmental communication via e‑mail; all clinical staff are required to check e‑mail accounts at the start of each shift.
- Effectively communicate, collaborate, and coordinate with colleagues and management on nursing process and service issues.
- Create and implement nursing care plans, reviewed daily and updated as needed.
- Consider financial implications in rendering cost‑effective nursing care.
- Ensure professional standards of nursing practice are maintained.
- Complete required documentation prior to departing from scheduled shift.
- Participate in the IDG (Interdisciplinary Group) and communicate patient needs appropriately to other disciplines when requested.
- Collaborate with team physician daily on the status of the patient.
- Perform other duties as requested by the Nurse Care Manager.
- Maintain up‑to‑date skill sets as required to care for patients.
- Possess ability to multi‑task; exhibit adaptability and flexibility to meet Gulfside Healthcare Services objectives.
- Ensure compliance with all patient care and standards and regulations.
- Assist in the implementation and maintenance of the policies and procedures of the clinical department; adhere to and assist in their evaluation.
- Assist in maintaining federal and state regulatory compliance.
- Proficient in Electronic Medical Records.
