• Conduct daily nursing functions in accordance with regulations and guidelines for long‑term care facilities.
• Ensure nursing personnel comply with written policies, including safety program.
• Admit, transfer, and discharge residents with proper documentation.
• Participate in orientation of new residents and families.
• Make rounds with health care providers, hospice staff, and home health staff.
• Utilize nursing personnel according to resident care plans and provider orders.
• Complete clinical rounds and report significant events.
• Resolve issues with residents and families; report to supervisor.
• Participate in facility surveys and inspections.
• Assist with clinical programs such as dementia, wound care, quality assurance, infection control, and admissions.
• Complete initial assessments and contribute to care plan development.
• Maintain accurate documentation of care plans, progress notes, medication orders, and diagnostic services.
• Communicate with providers regarding treatment plans, orders, and progress notes.
• Arrange diagnostic and therapeutic services as ordered.
• Communicate policies to personnel, residents, and visitors.
• Report discrepancies in orders, diets, or charting to the supervisor.
• Prepare and administer medications; ensure adequate medication supply.
• Dispose of drugs and narcotics per procedures.
• Supervise nursing unit; address performance issues with Director of Nursing.
• Maintain clean and sanitary nursing service areas and resident rooms.
• Recommend equipment and supplies to the Director of Nursing.
• Perform other duties as assigned.

• Must meet all applicable state and federal licensing requirements.
• Current, unencumbered, active LPN/LVN license in good standing with no disciplinary action.
• One year experience in assisted living or long‑term care (preferred).
• Ability to make independent decisions and follow instructions.
• Excellent communication skills with staff, residents, families, and agencies.
• Capability to work with ill, disabled, elderly, and emotionally upset individuals.
