Infectious Disease Nurse Practitioner (NP)
Elevate the standard of care for our geriatric population as a valued member of the Aviata Health Group team. As an in‑house NP, you will be the primary clinician for residents, leading the infectious disease program, building relationships with patients, families, facility staff, and external providers, and contributing to the clinical leadership of our skilled nursing and long‑term care facilities.

- Master’s Degree in Nursing from an accredited program
- Strong clinical background in infectious disease
- Geriatric or adult medicine experience preferred, with focus on skilled nursing, long‑term care, hospice, palliative care or similar settings
- Ability to engage and communicate effectively with elderly and chronically ill patients and their families
- Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target
- Knowledge of Medicare, Medicaid and health‑plan benefit structures is a plus
- Perform comprehensive physical examinations and evaluate changes in patient conditions
- Prescribe medications and treatments as needed
- Develop patient‑care policies and facilitate referrals to specialists
- Coordinate advanced care planning and discharge visits
- Ensure timely communication and documentation of patient assessments, treatment plans and follow‑ups
- Support the physician‑patient relationship and address gaps in care
- Annual pay: $105,000 – $130,000 based on experience
