Clinical Care Coordinator; LVN

Renew Home Health is looking for an organized, compassionate Clinical Care Coordinator (LVN) to help keep patient care running smoothly across our Cleburne office. In this full-time role, you’ll be the link between patients, families, clinicians, and physicians — coordinating schedules, orders, and communication so every patient gets the right care at the right time. • Coordinate patient schedules, visits, and clinician assignments
• Communicate with patients, families, physicians, and referral sources to keep care on track
• Process and follow up on physician orders and documentation
• Support the clinical team with timely, accurate charting and records
• Help ensure care is delivered in line with the plan of care and compliance standards
• Keep the office running smoothly and jump in wherever the team needs you

• Current Texas LVN license in good standing
• Home health experience a plus (coordination or scheduling experience especially welcome)
  Strong organizational and communication skills
• Comfortable with technology and electronic documentation
• A team player who stays calm and organized under pressure
