Primary Care Nurse Practitioner / Physician Assistant

Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in‑house support staff.

Position Summary

Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for the specialties of family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facility work.

Essential Duties and Responsibilities

Quality/Compliance

  • Review and sign MARs as requested by facilities, sign all Primary Care orders while at the facility and/or within 24 hours if faxed or emailed (including phone orders, DME orders, death certificates, plans of care, PT/OT orders, hospice orders and other relevant orders). Review and address labs within 24 hours of receipt. Manage all narcotic prescriptions for your facility’s patients in accordance with Eventus Whole Health Narcotic Rx Policy.

    Allocate adequate time each visit for paperwork and administrative duties.
  • Provide on‑call triage duties as per the on‑call triage protocol. Triage policies and scheduling will be modified as needed; all providers must adhere to the most recent schedule. Eventus Whole Health triage policies are based on a majority vote of all involved providers.
  • Complete documentation and submit billing records to Eventus Whole Health in accordance with published charting and bill‑sheet submission policies. Provide the facility with a hard copy of the electronic documentation within 48 hours of service for inclusion in the patient’s medical record.
  • Establish and maintain open, positive communication with facility staff and administration. Provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information. Meet with the facility administrator or designated care coordinator before the first patient visit each day and before leaving the facility each day.
  • Be knowledgeable of and adhere to Eventus Whole Health standards, policies, and procedures. Follow all legal, regulatory, and professional ethical standards applicable to health‑care delivery.
  • Maintain patient confidentiality at all times, including appropriate use of cell phones, emails, text messaging, patient charts, and EMR. Abide by HIPAA rules and regulations.
  • Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding resident care.
Qualifications / Education
  • Master’s degree in Nursing from an accredited institution and a current, valid state license to practice medicine. All other necessary certifications must be current and valid.

OR

  • Graduate of a physician assistant program accredited by a national commission on accreditation of allied education programs.
  • Passed the national certification examination administered by the accreditation commission.
  • Licensed as a Physician Assistant in the state of residence.
  • Specialization in geriatric, family, or psychiatry is desired.
