Baylor Registered Nurse, Home Health – HR role at Humana

- Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient; provide necessary follow‑up as directed by the Clinical Manager.
- Confer with physician to develop the initial plan of treatment based on physician's orders and initial patient assessment; provide hands‑on care, management and evaluation of the care plan, and teaching of the patient as per physician orders under Clinical Manager’s supervision.
- Coordinate appropriate care with other healthcare personnel (e.g., Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
- Report patient care/condition/progress to patient’s physician and Clinical Manager continuously.
- Implement patient care plan in conjunction with patient and family to achieve optimal resolution of needs/problems.
- Coordinate/oversee/supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide written personal care instructions and care plan; monitor documentation completion by aides as part of supervisory responsibility.
- Discharge patients after consultation with physician and Clinical Manager, preparing and completing required clinical documentation.
- Prepare appropriate medical documentation for all patients, including case conferences, patient contacts, medication order changes, re‑certifications, progress updates, and care plan changes; prepare visit/shift reports and summaries; confer with other disciplines to provide optimum patient care.
- Diploma, Associate or Bachelor Degree in Nursing.
- Minimum of one year of nursing experience preferred.
- Strong experience in med‑surg, ICU, ER, or acute care.
- Experience in home health (plus).
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
$28.00 – $40.00 per visit/unit; $44,600 – $61,400 per year base pay.

ScheduleFull‑time (40 hours per week), with a 32‑hour weekly schedule available.
