Job DescriptionThe Registered Nurse, RN, is a member of the interdisciplinary team who works together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN is responsible for care coordination, ongoing assessment of patient and family needs, and implementing the interdisciplinary plan of care.
Job Responsibilities- Case management and supervision of nursing staff and aides.
- Follow VITAS policies and standards, practicing within the scope of the RN license.
- Perform comprehensive assessments and provide education to patients and families.
- Participate in team meetings and communicate effectively.
- Participate in on-call rotations and provide care during non-business hours.
- Delegate tasks appropriately and evaluate outcomes.
- Contribute to quality improvement initiatives and professional development.
- Maintain documentation and ensure compliance with regulations.
- Current RN license in the state of practice.
- Minimum of two years' experience in hospice, home health, or community health.
- Reliable transportation and a working telephone.
Assist in patient lifting and transfers, stand or walk for extended periods, and perform vision and physical tasks as required.
Qualifications- Licensed to practice nursing in the state where the VITAS program operates.
- Experience in hospice, home health, or community health preferred.
