Job DescriptionThe Registered Nurse, RN, is a member of the interdisciplinary team responsible for meeting the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN provides coordination of care, continuous assessment of patients and families, and implements the interdisciplinary plan of care.
Primary Responsibilities- Case management and supervision of nursing staff and volunteers.
- Follow VITAS policies and standards, practicing within the scope of RN licensure.
- Perform comprehensive assessments and develop care plans.
- Provide patient education, including pain management.
- Participate in team meetings and bereavement support.
- Participate in on-call rotation and provide care during non-business hours.
- Delegate tasks appropriately and evaluate outcomes.
- Ensure compliance with healthcare regulations and infection control practices.
- Contribute to quality improvement initiatives.
- Engage in professional development and training activities.
- Maintain effective communication and serve as a patient and family advocate.
- Current RN license in the state of practice.
- Minimum of two years' experience in hospice, home health, or community health within the last five years.
- Reliable transportation and available communication means.
Must be able to assist in lifting and transferring patients, stand or walk for extended periods, and perform other physical activities as required.
Qualifications- Licensed to practice nursing in the relevant state.
- Experience in hospice, home health, or community health preferred.
