Your Role:
Specific Duties and Responsibilities

As an Internal Medicine Physician, you will:

  • Complex Clinical Care: Manage and improve health for patients with multiple chronic conditions.
  • Lead a multidisciplinary team, collaborating with social work, case management, and other specialists.
  • Develop strong transitional care plans, ensuring continuity from hospital to home.
  • Communicate effectively with various care facilities to facilitate appropriate patient discharges.
  • Treat acute exacerbations of chronic conditions within the Absolute Care center.
  • Accept urgent appointments to provide timely care for patients in need.
  • Monitor patients closely to prevent unnecessary ER visits and hospital admissions.
  • Educate members on chronic disease progression and the importance of advanced directives.
  • Practice evidence-based medicine in alignment with the Patient-Centered Medical Home (PCMH) model.

Candidate Qualifications

We are looking for candidates who possess:

  • A valid medical license in the state of practice.
  • Board certification in Internal Medicine or Family Practice.
  • BLS certification and DEA registration.
  • A mission-driven mindset to serve underserved populations.
  • The ability to work collaboratively within a multidisciplinary team.
  • Experience with electronic medical records (EMR).
  • Strong problem-solving skills and creativity in finding solutions.

Preferred Qualifications

While not mandatory, the following qualifications are a plus:

  • Experience in population health management.
  • Experience working with psycho-socially and medically complex patient populations.
