A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource to others in the work-team and department.
What You Will Do- Provides psychosocial assessment / intervention to assist patients/families (including clinically complex). Provides age specific care to patients/families in all age groups.
- Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. Develops plans with attention to individual patient and family goals. Provides linkage to community resources and UH network services to facilitate optimal care.
- Communicates/collaborates with the work team using a multidisciplinary approach.
- Provides guidance/consultation to peers/team regarding routine psychosocial issues and/or discharge planning issues.
- Provides supervision to social work interns if LISW eligible (completion of supervision hours).
- Performs other duties as assigned.
- Complies with all policies and standards.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- Master's Degree from an accredited school of Social Work (Required)
- Hospital/Health Agency experience. (Preferred)
- Licensed Social Worker (LSW) (Required)
- Licensed Independent Social Worker (LISW) (Preferred within 1 Year)
Previous experience working in a hospital setting is ideal. Prior work with pediatrics is strongly preferred. This position is for Pediatric Endocrinology. The social worker will assist with securing resources and helping address the psychosocial aspect of care found in pediatric endocrine patients. This position is part time, 20 hours per week. It is mainly located in Cleveland, but will be required to travel to other clinics.
