POSITION SUMMARY:
The Physical Therapist evaluates, plans, and delivers physical therapy services to residents in accordance with physician orders and professional standards of practice, focusing on mobility, strength, balance, and safe transition to the highest practicable level of function.
- Perform initial and ongoing evaluations and develop individualized PT plans of care.
- Deliver skilled physical therapy interventions to address mobility, strength, balance, gait, transfers, and pain.
- Educate residents, families, and caregivers in safe mobility and home programs.
- Document evaluations, daily notes, progress notes, and discharge summaries in compliance with Medicare and state requirements.
- Collaborate with the IDT on care plans, discharge planning, and restorative carryover.
- Supervise PTAs and therapy aides where applicable.
Active Physical Therapist license in the state of practice.
