Overview
The Department of Medicine at the VA Northeast Ohio Healthcare System is seeking a full-time (APN) Nurse Practitioner within the Pulmonary and Critical Care section of Medicine Service. This position provides a full range of services emphasizing quality, timeliness, and the advancement of the VA mission. The (APN) Nurse Practitioner provides advanced clinical practice, consultation, and management.
Responsibilities- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with the interdisciplinary team.
- Orders and interprets labs, tests, and diagnostic studies.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals and consults to specialty services.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
- Obtain clinical histories and perform physical examinations on patients with a variety of complex pulmonary conditions.
- Collects and synthesizes patient data to formulate comprehensive patient care plans, including formulation of differential diagnoses, prescribing treatment modalities, and ordering diagnostic tests to determine effectiveness of treatment modalities.
- Completes outpatient clinic, e-consult, and telehealth patient encounters.
- Core responsibilities include pulmonary disease consultation and triage, receiving and responding to electronic and phone communication with pulmonary patients, and collaboration with the clinic scheduling team.
- Tracks performance measures and utilizes data to enhance access, implement new projects, and improve existing ones.
- Creates and/or develops best practice standards.
- Interfaces with trainees (Pulmonary Fellows, Medicine Residents, Respiratory Therapy students, Pharmacy Residents, and Nurse Practitioner students) in a multidisciplinary setting and serves as a resource for these learners; also serves as a basic pulmonary resource for other providers/services at the Medical Center.
- Training will be provided as needed; other duties may be assigned at the discretion of Pulmonary Section leadership.

- APN Nurse Practitioner (pulmonary/critical care) with appropriate licensure and certification.
- Experience in pulmonary care and comfort working in a VA health care system setting; ability to work at multiple sites (Louis Stokes VA Medical Center and Parma - OH CBOC).
- Ability to work collaboratively with an interdisciplinary team and supervise/educate learners, as applicable.
- Clinical history taking and physical examination with complex pulmonary conditions; strong critical thinking and decision-making skills.
- Licensure: 1 full and unrestricted license from any US State or territory.
- Schedule flexibility and ability to participate in a range of clinical activities including outpatient clinics, telehealth, and e-consults as needed.
