Affinity Health Management is seeking an experienced Registered Nurse for hospice home health care. The RN will assess patients and families, plan and deliver nursing care in the home setting, and collaborate with the interdisciplinary team to meet individualized goals.
Responsibilities include documentation, care coordination with physicians, patient education, and participation in quality improvement activities. Ohio licensure and recent hospice/home health experience are preferred.
