Licensed Practical Nurse – Sevita

Location:

Cleveland, TN |

Employment Type:

Full-Time (also part-time and PRN available)

Pay: $20.40 hourly (Full-time) / $25.00 hourly (PRN)

Schedule:

Rotating 7:00 AM – 7:00 PM. Week 1: 3 days, Week 2: 4 days. PRN: 2 days/week and 1 weekend/month.

D&S Community Services, part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities.

Responsibilities
  • Monitor health conditions of individuals served and ensure quality services.
  • Administer medications, provide documentation, and oversee staff medication administration.
  • Provide prescribed medical treatments and personal care services.
  • Conduct assessments and implement health‑care components in Individual Service Plans (ISP); respond to condition changes.
  • Educate staff on universal precautions, infection control, and medication administration.
  • Liaise with medical professionals and external agencies to coordinate consistent treatment.
  • Provide training to individuals served on self‑medication, prescribed treatments, sex‑education/STD prevention, and smoking cessation.
Qualifications
  • Associate’s Degree in Nursing or related field, or state certificate.
  • One year of nursing experience with the population served.
  • Current state LPN or LVN license for state of residence.
  • Valid driver’s license, car registration, and auto insurance.
  • Required certifications per state regulations (e.g., CPR/FA, HIV/AIDS certification, BBP, ACT training).
  • Effective communication with staff, individuals served, and families.
