Title
Physical Therapist (PT) PRN - Home Health

SummaryAdminister intermittent physical therapy in patients' homes under physician orders, developing and evaluating care plans, supervising assistants, and participating in QAPI program.
Responsibilities- Provide physical therapy to patients in their residences per physician orders.
- Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
- Offer patient, caregiver, and family counseling and education.
- Supervise Physical Therapy Assistants (PTA) and Home Health Aides (HHA) as needed.
- Communicate with all physicians involved in the plan of care and other healthcare practitioners related to the current plan of care.
- Contribute to all phases of QAPI program and participate in ongoing interdisciplinary assessments of patients.
- Active Physical Therapist (PT) license in the state(s) of practice.
- Two (2) years experience, Home Health preferred.
- CPR certified with American Heart Association or American Red Cross.
- Strong communication skills, both oral and writing, with excellent interpersonal skills.
- Good customer relations skills and commitment to providing quality service.
- Support the core values and goals of Agency.
- Flexibility, initiative, and work with minimal supervision.
- Current valid driver's license, auto liability insurance, and reliable transportation.
- 100% travel required.
Physical Requirements
- Work in patient homes and travel to/from patient homes, requiring significant physical effort including kneeling, squatting, reaching, twisting, climbing, walking, and exposure to temperature and humidity changes.
- Assist in lifting and/or transferring a 100-pound patient.
- Must possess appropriate sight and hearing, or adaptive devices, to meet job duties.
- Provide annual TB test and other state-required medical examinations.
- Be able to tolerate exposure to blood, body fluids, excrements, and hazardous materials.
