• Assess & plan:
  Perform assessments and contribute to individualized care plans.
• Medications & treatments:
  Administer per orders; monitor responses and side effects.
• Clinical monitoring:
  Track vitals, changes in condition, and escalate promptly.
• Communication & education:
  Support residents and families with clear, timely updates.
• Teamwork:
  Collaborate with RNs, CNAs, therapy, and providers for smooth transitions of care.
• Documentation:
  Complete accurate, and timely notes in the EMR.
• Professional growth:
  Participate in ongoing training and competencies.

• Active Texas LVN license in good standing. (Required)
• 1+ year of nursing experience in SNF/LTC preferred (new grads welcomed with strong references).
• Strong clinical judgment, organization, and communication; reliable and team-oriented.
