Licensed Vocational Nurse (LVN) – In Home Care

Adaptive Home Health is a privately owned agency delivering high‑acuity, patient‑centered care throughout Texas. We are seeking elite Licensed Vocational Nurses (LVN) with previous home health experience to provide comprehensive care in the patient’s home. This position is full‑time with an expectation of 30 productivity points completed per week. Clinicians are paid per point and may earn additional pay by exceeding 30 points.

• Treat wounds, administer medications and treatments as prescribed, provide infusion therapy, wound care, teaching, and disease management.
• Serve as patient advocate and coordinate care with physicians, referral sources, caregivers, and facility management.
• Maintain up‑to‑date documentation through Homecare Homebase.
• Participate in regular case conferences and in‑services.
• Provide care to approximately 5 patients per day, Monday–Friday.
• Organize and manage time effectively, multi‑task, and meet timelines.
• Maintain professional appearance and effective communication skills.

• Current Texas LVN license.
• Three years of clinical nursing experience.
• Current Texas driver’s license.
• CPR certified.
• Experience with healthcare software.
• Competence to perform any skilled procedure within the scope of the Nurse Practice Act.
