Licensed Practical Nurse; LPN Continuous Care NIGHTS

Join VITAS Healthcare as a Licensed Practical Nurse (LPN) for Continuous Care during NIGHT shifts. This role involves providing compassionate nursing care to terminally ill patients in accordance with state Nurse Practice Acts and the established care plan.

• Provide direct skilled bedside nursing care.
• Evaluate patient and family needs, assisting in the development and execution of interdisciplinary care plans.
• Perform selected nursing acts under RN supervision, including administering treatments and medications.
• Monitor and evaluate the effectiveness of treatments and patient reactions.
• Report significant findings and changes in patient condition to the RN, Team Manager, or Physician, and document appropriately.
• Maintain a safe, clean environment for patients and families.
• Ensure accurate documentation of clinical notes and interdisciplinary charts.
• Establish therapeutic relationships with patients and families, encouraging participation in care plans.
• Implement nursing protocols related to patient comfort and mental well-being.
• Participate in patient and family education about illness, treatment, and self-care.
• Engage in ongoing professional development and participate in research activities as assigned.
• Participate in on-call rotations and be flexible with work schedules, including weekends and rotating shifts.

• Graduate from an approved Practical/Vocational Nursing program.
• Current licensure to practice as an LPN in the state of employment.
• At least one year of satisfactory nursing experience, preferably in a hospital adult medical-surgical unit.
• Reliable transportation with required liability insurance and license.

Must be able to assist with patient lifting and transfers, stand or walk for extended periods, and perform physical tasks as needed. Visual and physical dexterity are also required.
