About the Role:Serenity Hospice Care is seeking a compassionate, dependable Licensed Practical Nurse (LPN) to join our team as a Full-Time Weekend Baylor providing overnight hospice continuous care. As a Hospice LPN Continuous Care, you will provide high-quality, patient-centered care to hospice patients requiring continuous care throughout Burlington, Camden, and Mercer Counties.
Responsibilities:Provide nursing care according to the patient's plan of care and under RN supervision
Administer medications and treatments as ordered and within the LPN scope of practice
Monitor patient comfort, symptoms, and condition, promptly reporting changes to the supervising RN
Support patients and families with compassion, dignity, and respect
Complete accurate, timely clinical documentation
Communicate with the hospice clinical team to support continuity of care
Be willing to do patient visits and wound care when Continuous Care cases are not needed
Current, active New Jersey LPN license
Strong clinical, communication, and documentation skills
Ability to work all three required overnight shifts (Friday: 7:00 p.m.
-8:00 a.m. Saturday, Saturday: 7:00 p.m.
-8:00 a.m. Sunday, Sunday: 7:00 p.m.
-8:00 a.m. Monday)Reliable transportation
Hospice, palliative care, or long-term care experience preferred
