Program Counselor; Case Manager

Program Counselor (Case Manager) – Vista Life Innovations – Clinton, CT

We are seeking a Program Counselor (Case Manager) to join the Vista team to coordinate, monitor, and document all counseling and case management services provided to our members. This Full-Time and Benefits and Paid Time-Off
-eligible position will be based at our Clinton, CT community, supporting members who reside between Guilford and Old Saybrook.

• Develop written program plans (IPP’s and IP’s) for each member that evaluate progress, determine individual goals, and identify programmatic service needs.
• Implement systems supporting each assigned member’s programmatic and individual goals addressing four main areas: social/emotional, vocational, life skills, and community involvement.
• Provide individual and family counseling for members.
• Communicate member needs, issues and goals to all staff members and other involved parties.
• Maintain all necessary records and documentation, including monitoring members’ medical, wellness, and financial needs.

• Bachelor’s Degree plus one (1) year of related experience. Vista values the work experience of applicants and may waive the Bachelor level of educational requirement of a position in lieu of a strong work background in a related field.
• Valid driver’s license plus a safe driving record.
