Currently seeking a Physical Therapist- PRN for a position that is available in a long-term care/skilled nursing facility. • Perform comprehensive evaluations of residents to assess strength, mobility, balance, coordination, and functional ability.
• Develop and implement personalized physical therapy treatment plans based on clinical findings and physician orders.
• Provide therapeutic interventions including exercise, gait training, manual therapy, pain management, and use of modalities.
• Document all evaluations, treatments, and resident progress in accordance with regulatory, facility, and professional standards.
• Collaborate with nurses, physicians, occupational therapists, speech therapists, and other members of the healthcare team to ensure coordinated care.
• Comply with all federal, state, and facility policies including HIPAA, Medicare/Medicaid documentation guidelines, and infection control protocols.

• Must be a graduate of a accredited therapy program (Bachelor Degree required. Masters Degree Preferred)
• Must possess or be eligible for state licensure
• Some previous inpatient rehab or LTC/SNF experience preferred but not required.

• Competitive salaries $39.00/hr – $58.00/hr Depending on experience
