Education and Outreach Specialist – PORT Health
OverviewEasterseals PORT Health (ESPH) is seeking a compassionate Education and Outreach Specialist to join our multidisciplinary One Community team in the Southern Region of the Trillium service area. This role focuses on increasing access to care, improving health literacy, and promoting wellness through community education and engagement. The Education and Outreach Specialist serves as a bridge between community members and providers through outreach and education across the southern Trillium counties on the East coast of North Carolina.
This is a community-based position and candidates must reside in Bladen County. Traveling to Jones, Onslow, Pender, New Hanover, Brunswick, Bladen, or Columbus counties may be required. The position may involve evenings, weekends, and holidays to support community events. Flexibility is essential to accommodate activities outside standard business hours.
Management preference: Experience with the unhoused population and/or community-based experience.
Responsibilities- Attend community events (evenings, weekends, holidays as needed) to promote education and connect individuals to resources.
- Engage with community referrals and maintain working relationships with local providers.
- Support the team by managing multiple projects and maintaining attention to detail.
- Travel to specified Trillium counties as part of a regional outreach program.
- High School diploma or GED required
- Valid NC driver’s license and auto insurance for personal and agency vehicles used for work
- Hourly rate of $18
