Job Duties / Responsibilities- The APS Case Manager performs specialized case management in the area of Adult Protective Services (APS), Division of Adult Advocacy, in Spartanburg County. Responsibilities include investigations, assessments and coordinating protective and preventive services for vulnerable adults. May assist other counties with APS case management as needed.
- Investigates to determine vulnerability, abuse, neglect by a caregiver, self-neglect and exploitation reports of vulnerable adults, while respecting self-determination. May assist other counties with APS case management as needed.
- Provides case management services according to APS policy and procedures, including home visits and participation in legal proceedings; documents in CAPSS in a timely and accurate manner.
- Coordinates necessary services to keep the vulnerable adult safe, including temporary and long-term placements.
- Assists law enforcement and/or family court in emergency removal of adults and securing placement in a protective environment.
- Maintains professional and harmonious relationships with co-workers and the community.
- Assesses the vulnerable adult's ability to secure long-term services, including but not limited to redirecting funds, securing a guardian or conservator, filing appropriate documents for emergency services, and connecting to probate or family court.
- Ensures that Personal Identifying Information (PII) is properly used, accessed, gathered, shared and disposed; protects agency networks, systems, and data.
- Performs Red Cross shelter duty or other emergency shelter duties as directed.
- Performs other duties as required and/or directed.
- A Bachelor’s Degree in Social Work, Human Services, Public Administration or a related field.
- A Bachelor’s Degree in any other field and one year of experience in a related field.
- Must possess a valid driver’s license and have access to and be able to operate a motor vehicle. Individuals must comply with necessary field work assignments.
- If certified, you possess educational credentials that qualify you for available DSS positions; a certified official transcript will be required if you are selected for a job offer.
