Private Duty LPN for Home Care

Location: Clinton, TN (available hours 8 a.m. – 8 p.m.)

Job Summary:

The Home Care Licensed Practical Nurse is responsible for providing direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes, recording clinical and progress notes, and coordinating care activities to provide the highest patient care. The role is supervised by a Registered Nurse or designated supervisor and all nursing activities comply with the State LPN/LVN Practice Act, licensure and certification requirements, and agency policies.

Specific Responsibilities:

• Provide high‑quality clinical services within the scope of practice per state laws.
• Document patient visits thoroughly in the electronic medical record.
• Participate in care‑coordination activities: report patient needs, attend case conferences, and communicate effectively with patients, families, physicians, clinicians, and case coordinators.
• Maintain confidentiality of patient health information.

Education and Experience:

• Current LPN licensure in Tennessee.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
• Previous home‑care experience preferred.
