- Provide direct patient care and support in rehabilitation settings.
- Develop and implement individualized care plans for patients.
- Monitor patient progress and adjust care plans as needed.
- Participate in ongoing professional development and training.
- Assist in achieving departmental performance targets and metrics.
- Collaborate with interdisciplinary teams to enhance patient outcomes.
- Educate patients and families on rehabilitation processes and goals.
- Adhere to safety and infection control protocols at all times.
- Maintain accurate and timely documentation of patient care.
- Provide emotional support and counseling to patients and families.
- Engage in quality improvement initiatives to enhance care delivery.
Participate in staff meetings and contribute to team discussions.
- Current RN license with at least 2 years of rehabilitation experience.
- Proven ability to develop and implement effective care plans.
- Commitment to ongoing professional development and training.
- Ability to monitor patient progress and adjust care plans effectively.
- Strong communication skills for patient and family education.
- Experience with interdisciplinary team collaboration in healthcare settings.
- Demonstrated ability to provide emotional support to patients and families.
- Familiarity with safety and infection control protocols.
- Experience in maintaining accurate and timely documentation.
- Willingness to engage in quality improvement initiatives.
