Registered Nurse - Rehabilitation Center - RN - LTC / SNF

Talented Medical Solutions is looking for a Registered Nurse to work in a Rehabilitation Center (LTC/SNF). This role is based in Clovis, NM and is a 12‑hour per shift, days shift position working 7 weeks contract.

Responsibilities
  • Provide direct patient care and support in rehabilitation settings.
  • Develop and implement individualized care plans for patients.
  • Monitor patient progress and adjust care plans as needed.
  • Participate in ongoing professional development and training.
  • Assist in achieving departmental performance targets and metrics.
  • Collaborate with interdisciplinary teams to enhance patient outcomes.
  • Educate patients and families on rehabilitation processes and goals.
  • Adhere to safety and infection control protocols at all times.
  • Maintain accurate and timely documentation of patient care.
  • Provide emotional support and counseling to patients and families.
  • Engage in quality improvement initiatives to enhance care delivery.
  • Participate in staff meetings and contribute to team discussions.
Qualifications & Experience
  • Current RN license with at least 2 years of rehabilitation experience.
  • Proven ability to develop and implement effective care plans.
  • Commitment to ongoing professional development and training.
  • Ability to monitor patient progress and adjust care plans effectively.
  • Strong communication skills for patient and family education.
  • Experience with interdisciplinary team collaboration in healthcare settings.
  • Demonstrated ability to provide emotional support to patients and families.
  • Familiarity with safety and infection control protocols.
  • Experience in maintaining accurate and timely documentation.
  • Willingness to engage in quality improvement initiatives.
