LVN​/LPN – LTC​/SNF

Job Summary:

Seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide high-quality patient care in a long-term care and skilled nursing facility. The role involves supporting patient health, administering treatments, and collaborating with healthcare teams to ensure optimal outcomes.

Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patient vital signs and report changes accurately.
• Assist with patient admissions, discharges, and daily activities.
• Document patient care activities thoroughly and promptly.
• Collaborate with interdisciplinary team members to develop and implement care plans.
• Adhere to all safety and infection control protocols.

Qualifications:

• Valid LVN/LPN license in the state of New Mexico.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to work flexible days, including Sundays, Fridays, and Saturdays.
• Excellent communication and interpersonal skills.
• Ability to wear a photo badge and present required identification.
• Compliance with COVID-19 vaccination requirements or valid exemption documentation.
