• Administer medications and document per provider orders
• Monitor residents, assess condition changes, and report to the care team
• Provide foundational nursing care, wound care, and daily hygiene support
• Assist with activities of daily living and mobility needs
• Maintain accurate documentation of vitals, treatments, and care plans
• Collaborate with interdisciplinary team to ensure safe, patient-centered care for residents

• Active Licensed Practical Nurse license in New Mexico
• Ability to provide care in a skilled nursing facility/long-term care setting
• Availability to work the assigned schedule (3x12-hour day shifts, 06:00–18:30)
• Commitment to patient safety, accurate documentation, and adherence to facility policies
