District Nurse - District Nursing Services​/Community Nursing

An exciting opportunity has arisen within the West Dunbartonshire District Nursing Team for a District Nurse Specialist Practitioner. This rewarding and dynamic role delivers high‑quality, person‑centred care to patients in their own homes and communities. As an autonomous practitioner, you will lead and support the district nursing team, ensuring safe, responsive and effective care across a diverse caseload and work closely with multidisciplinary partners to coordinate care and improve patient outcomes.

  Monday to Sunday, 4 × 9 hours

• Lead and manage a defined district nursing caseload, ensuring safe, effective, person‑centred care delivery.
• Provide skilled clinical interventions and act as the clinical expert within community nursing practice.
• Coordinate workload, prioritise care needs and respond to both planned and urgent unscheduled requests.
• Work collaboratively with multidisciplinary teams, patients and carers to plan and deliver holistic care.
• Delegate, supervise and support team members to maintain high standards of practice and service delivery.
• Maintain accurate clinical records and ensure compliance with professional, legal and organisational standards.
• Contribute to service improvement, audit, education and the ongoing development of the district nursing team.

• Registered Nurse with current NMC registration in Adult Nursing.
• Specialist Practitioner Qualification in District Nursing at Master’s level or equivalent.
• V300 Independent Prescribing qualification and advanced clinical assessment skills.
• Demonstrable leadership and management experience within a community or primary care setting.
• Evidence of continuous professional development and commitment to lifelong learning.
• Excellent communication, problem‑solving and organisational skills with the ability to work autonomously and within a team.
• IT skills and ability to use electronic patient record systems alongside a full driving licence and access to a vehicle.
