Ava Med Workforce is a Healthcare Staffing Agency that staffs all of California State Correctional Facilities. We are in URGENT need of a Physical Therapist to take on a full‑time assignment at the correctional facility located in Coalinga, CA
. This is a low‑stress setting where you have full clinical autonomy, no rug rates, no after‑hours documentation, and no productivity quotas.

Schedule:

Monday‑Friday 7:00‑3:30pm (1‑2 weeks of orientation required)
Rate: $85/hr ($3,400/week)
Status:
Independent Contractor (1099)

Requirements

Physical Therapist license by the California State Board of Physical Therapy.

Job Description

Perform care consistent with the scope of licensure and in accordance with the Institution/Facility policies and procedures, and generally accepted evidence‑based physical therapy practices.

Provide physical therapy and evaluations on an "as needed" basis as pre‑scheduled by the designated Institution/Facility designee.

Provide physical therapy services to patients/youth upon physician referral or order, or direct access for patients with an existing documented diagnosis.

Provide an initial assessment/evaluation on each referral, develop a treatment plan, and communicate the plan to the patient/youth and relevant medical personnel.

Report assessments/evaluations and progress notes using approved documentation forms.

Vendor worker shall be available for telephone consultation while on duty at the Institution/Facility.

Maintain charting daily; any chart delinquent for more than ten days may result in termination.
