The Care Coordinator will support both the clinical and administrative functions of the organisation to enhance patient care and service delivery. They will work closely with a range of primary care professionals to manage identified patient caseloads, supporting patients andcarers before and after clinical consultations, and ensuring evolving needs are addressed. In addition, they will coordinate the effective running of clinics, including managing Systm One schedules,advertising shifts, supporting service mobilisation, and maintaining accurate data for governance and reporting.

The coordinator will also facilitate MDT coordination, manage patient pathways by arranging investigations and follow-up appointments, and ensure timely treatment in line with national standards.

Main duties of the job

Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.

Help people to manage their needs, answering their queries and supporting them to make appointments.

Ensure that people have good quality information to help them make choices about their care.

Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.

General Administration and note taking as appropriate.

Manage the adding and editing of clinics on Systm One.

Respond to and directing telephone calls, including taking messages when required.

Monitor the Federation email accounts.

Support the Federation management team in the collection and submission of relevant data.

Arrange appointments for investigations and/or follow up appointments for additional patient tests as required by the clinician.

Assist the effective management of the patient pathway ensuring that all necessary appointments, including the booking of outpatient clinics and waiting lists, are arranged on time and that patients receive timely treatment in accordance with national standards.

About us

North West Leicestershire GP Limited is a federation of 12 General Practices who work collaboratively to offer services at scale for our patients. The Federation is responsible for the running of the North West Leicestershire Primary Care Network which offers services to approximately 116, 000 patients.

Interviews for this role are likely to take place week commencing 30th March 2026.

Job responsibilities

To provide a Care Coordinator resource for healthcare professionals and service users, working in collaboration with other members of the Multidisciplinary Team across the Primary Care Network (PCN).

Working closely with healthcare professionals within the PCN to develop effective working relationships and identify and support a defined caseload of patients.

Supporting the creation of personalised care and support plans which are patient centred.

Providing coordination and navigation for patients and their carers across health and care services, plus other external agencies, so that their care plan is optimized and acted on.

Providing clear communication to patients and their carers to ensure that people have good quality information to help them make choices about their care and the services the PCN offers.

Using Systm One, and other approved data collection resources, to obtain and utilise practice data support patient care and the delivery of services.

Maintain a good working relationship with the practices within the PCN.

Provide a comprehensive Clinic Coordinator resource across all PCN/Federation services and contracts, supporting the coordination and delivery of MDTs within PCNs.

To ensure the effective running of all clinics.

Support the Federation team with the mobilisation of new services.
