Community Staff Nurse – South & City Community Nursing Team

We are seeking a motivated and compassionate Community Staff Nurse to join our friendly and supportive South & City Community Nursing Team within Bradford District Care NHS Foundation Trust.
Working within our Planned Care Service, you will provide high-quality, person-centred nursing care to individuals with a wide range of health needs, including long-term conditions, complex wound care and palliative care. As part of a multidisciplinary team, you will assess, plan, deliver and evaluate care while promoting independence and self-management. The service operates across a seven-day rota to ensure patients receive the support they need when they need it.
This is an excellent opportunity to develop your community nursing career, with access to ongoing professional development, clinical supervision and specialist training opportunities. Applicants must hold current NMC registration and be able to travel extensively across the district to undertake patient visits.

Join us in delivering outstanding care closer to home and help shape the future of community services across Bradford.
To work as part of the Community Nursing Team to ensure high-quality healthcare is delivered to patients in their own homes and/or clinic settings. You will work collaboratively with the wider Primary Health Care Team, participating in the assessment of health needs and the delivery of care to the local population. The service operates over a seven-day rota, and postholders are expected to work flexibly in line with agreed operating hours to meet service needs.
Key Responsibilities Include- Assessing, planning, implementing and evaluating nursing care for patients with a wide range of health needs.
- Delivering a variety of clinical interventions, including wound care, complex nursing treatments and medication administration.
- Working collaboratively with GPs, district nurses and multidisciplinary teams to coordinate care and improve patient outcomes,
- Promoting independence and supporting patients and carers to manage their own health and wellbeing wherever possible.
- Maintaining accurate clinical records and contributing to care planning, audits and service improvement initiatives.
- Supporting students and new team members while contributing to a positive learning environment.
- Prioritising workload effectively and responding to changing patient needs across community settings
