Job description

The Outreach Specialist will play a crucial role in identifying, engaging, and connecting eligible individuals to behavioral health programs including Substance Abuse Treatment, Psychiatric Rehabilitation Program (PRP), and Mental & Therapeutic Support (MTS) services. This role requires a proactive, empathetic professional who enjoys working directly with clients and community partners to build trust, support recovery, and strengthen program participation.

Responsibilities
  • Conduct outreach and engagement activities to identify and connect eligible individuals with programs such as Substance Abuse Treatment, PRP and MTS services.
  • Build and maintain positive relationships with clients, families, community partners, and referral sources.
  • Assist in client intakes, ensuring completion of required documentation and coordination of services.
  • Provide follow-up support to ensure clients remain actively engaged in treatment and rehabilitation programs.
  • Collaborate with counselors, case managers, and clinical staff to develop and implement individualized service plans.
  • Participate in community events and agency outreach initiatives to promote program awareness.
  • Maintain accurate and timely records of outreach activities, client contacts, and service outcomes in compliance with agency and COMAR standards.
  • Identify barriers to service participation and work with the team to develop strategies that support client access and retention.
  • Support clients in connecting with community resources such as housing, employment, education, and health services.
  • Attend team meetings, training, and supervision sessions to enhance professional growth and program effectiveness.
