Temah Family practice is looking for a Family Nurse Practitioner or an Adult Nurse Practitioner who is willing to work part-time or full-time.

• Diagnosing and treating acute illnesses, infections, and injuries such as medical illness.
• Management and treatment of common chronic conditions such as medical illness
• Telemedicine/telephonic consultations
• Chronic Care Management of medical illnesses
• Diagnosing and Treating substance use disorder including opioid use disorder
• Medication management including suboxone and monthly vivitrol
• Ordering, performing, and interpreting diagnostic studies.
• Medication management and reconciliation
• Collaborating with physicians, mental health team, and other health professionals
• Referral of patients to specialist as indicated including mental health
• Referral of patients to physical, occupation, and speech therapy (worked with Baltimore Ortho)
• Provides health education patients and families and connecting them to necessary resources
• Coordinating care to contribute to reduction in re-hospitalization
• Patient Advocacy
• Administer age-appropriate immunization
• Perform other duties as assigned
