Canopy Village Inc is seeking a Family Case Manager to engage families, assess needs, and coordinate resources across youth and guardians within the Canopy Village system. You will develop client-centered plans and connect families to respite, shelters, hospitals, and community services as needed.
Under supervision, you will collaborate with partners and treatment teams, facilitate groups for youth and parents, and drive proactive outreach to support at-risk families in our community.
